Endoscopic Forehead Lift and Revision Buried Suture Double Eyelid Surgery, Sub-brow Lift: Restoring Eye Structure at SH Plastic Surgery

This case study analyzes a patient with a narrow brow-to-eye distance, severe upper eyelid skin sagging, and forehead descent. We examine the 4-month post-operative results of an endoscopic forehead lift to relieve downward pressure on the forehead and eyebrows, combined with revision buried suture double eyelid surgery and upper eyelid surgery, to achieve a natural eye shape.

This is a case of a patient who presented with a narrow distance between the eyebrows and eyes, severe upper eyelid skin sagging, and forehead descent, leading to significant pressure on the eyes. We analyze the results 4 months after an endoscopic forehead lift, which alleviated the downward pressure on the forehead and eyebrows, combined with revision buried suture double eyelid surgery and upper eyelid surgery, to restore a natural eye shape. We explain an eye surgery approach focused on structural restoration rather than simple line correction. Director Kim Sung-hoon, SH Plastic Surgery, Face Designer

Hello, I am Director Kim Sung-hoon of SH Plastic Surgery.

The patient had previously undergone buried suture double eyelid surgery, and over time, the line had partially loosened.

However, the patient stated that she actually preferred the naturally loosened look of her eyes as they were.

She wished to improve the overall stuffy appearance of her eyes due to the seemingly narrow distance between her eyebrows and eyes,

so she sought a consultation.

The consultation revealed that a forehead lift was necessary to raise the eyebrow position and widen the distance between the eyes and eyebrows. Since a forehead lift could potentially cause the double eyelid to loosen further,

we planned revision buried suture double eyelid surgery concurrently to maintain a natural eye shape.

This patient had significant forehead descent, and her eyebrows were pressed down.

As a result, upper eyelid skin sagging appeared more prominent,

and insufficient strength to open the eyes led to pressure on the eyes.

Many people in this situation only consider upper eyelid surgery.

However, the starting point of the problem was ‘eyebrow position’.

Downward pressure due to forehead descent

Lowered eyebrow position

Structural instability of existing double eyelid line

The forehead muscles and corrugator supercilii muscles continuously exert downward pressure,

causing the eyebrows to descend and exacerbating upper eyelid skin sagging below them.

This leads to pressure on the eyes.

The eyes open, but not sufficiently.

This is why the distance between the eyebrows and eyes appears narrow.

Surgical Strategy — Releasing the Structure

Endoscopic Forehead Lift

The forehead is lifted through an incision inside the hairline.

This resolves forehead descent and induces natural bilateral movement of the eyebrows.

The key here is not ‘excessive lifting’ but normalization of the structure.

Revision Buried Suture Double Eyelid Surgery

The structure of the line is reorganized without excessive incision.

This reduces pressure on the eyes and creates a natural eye shape.

Concurrent Upper Eyelid Surgery if necessary

4-Month Post-Surgery Changes

The true results are visible after 4 months.

Normalization of previously covered pupil exposure

Reduced upper eyelid skin sagging

Improved distance between eyebrows and eyes

Resolution of sleepy-looking impression

Importantly, the eyes became clearer even without intentionally opening them wide.

Endoscopic forehead lift is not just a cosmetic surgery,

but a surgery that restores the functional opening of the eyes.

Director Kim Sung-hoon's Reflection, Face Designer

Eye surgery fails if you only look at the eyes.

Upper eyelid surgery alone often cannot solve the problem.

Sub-brow lift, endoscopic forehead lift, and revision buried suture double eyelid surgery

are not competing surgeries.

They are chosen based on the location of the problem.

If the distance between the eyebrows and eyes appears narrow,

it's not simply a matter of cutting skin.

If the structure is compressed, the structure must be released.

I always ask,

‘What is the reason these eyes are compressed?’

If that question cannot be answered,

no matter how precise the incision, it cannot solve the fundamental issue.

FAQ

Q1. If the distance between the eyebrows and eyes is narrow, is a sub-brow lift always necessary?

No. If forehead descent is the cause, an endoscopic forehead lift is prioritized.

Q2. Does an endoscopic forehead lift leave scars?

It is performed with an incision inside the hairline, so there is almost no external exposure.

Q3. Can't I just have upper eyelid surgery?

If forehead descent is the cause, it could actually make the eyes appear more stuffy.

Q4. Why was revision buried suture double eyelid surgery necessary?

Because the structure of the existing line was not stable.

Q5. Is insufficient strength to open the eyes also improved?

When structural pressure is relieved, functional improvement accompanies it.

Q6. How long is the recovery period?

Significant swelling subsides in 2-3 weeks, and natural results are achieved in 3-4 months.
